Shannon Christy's death here helped focus the thoughts of many an 'expert' kayaker on kayaking, risk, and loss.
A very good synopsis by Steve Fisher
and here
Shannon was an experienced kayaker who paid the ultimate price for a mistake.
Every kayaker needs to ask a few basic questions before every drop:
Can I see my line ? Can I hold my line ? Can I accept the consequences if I don’t ? I walk when I don’t “feel it”
Steve Fisher sums up by saying:
"...the adventure, the camaraderie and the sense of accomplishment is there
to be had, but so is the worst case scenario – the unexpected. If we’re
mindful and prepared for that, we’ll be just fine, ...We still get to run
the shit, but let’s step back and be better at not pitting our passion
directly against death, because when the scale tips the wrong way it’s
not worth it. "
The accident is still under investigation. No one saw how Shannon got into difficulty they just saw the empty, overturned kayak at the bottom of the run.

Shannon herself had said:
'We
have all been in situations where you've had to pull your spray skirt
and be ejected form the kayak,' she said. 'It's not something that's
ideal or what you want to do, but it does happen to the best of the
best.'
She said this in the actual interview with 60 minutes found here
